Each Employee must fill out the following forms
W-4 Form- This form is filed with both the IRS and the State of Tennessee. The W-4 disloses how much the employer will withhold on behalf of the employee for Federal Income Taxes. Here is the link to the form: https://www.irs.gov/pub/irs-pdf/fw4.pdf
W-2 Form – This form is filed with the IRS annually and given to the employee disclosing the employee’s wages for their personal income taxes. https://www.irs.gov/forms-pubs/about-form-w-2
I-9 Form – Every employee is required to fill out an I-9 Form which discloses that people are eligible to work in the United States. Employers must keep these documents on file for up to three years after the initial hiring date or 1 year after termination whichever is longer. https://www.uscis.gov/i-9
1095-B Form- This is the required form for each employee listing them and their dependents and the months during which they had health insurance.
